Martin, have you tried any other SSRI's other than zoloft? Cymbalta is SNRI - it hits more neurotransmitters than an SSRI, so more chance of side effects. I'd save Cymbalta for last resort, that is my personal opinion, you may be fine with it but my good friend had a severe reaction to it. Everyone is different tho. Try something gentler like Lexapro (once again my personal opinion). Have a chat to your doctor and ask what to expect when starting a new AD, duscuss and concerns you have. If concerned start at a low dose and work upwards.

Me out the door I can get any meds cause nhs but I havnt started taking these yet I was on citalopram at first but it screwed me right up ended up in hospital I just want some release while I await cbt lol cheers anyway
